Photo Release form for minors

I give Bestie Glam Squad Mobile Spa permission to use my child's likeness, image, voice and or appearances as such may be embodied in any pictures, photos video recordings, digital media, and the likeness taken or made on behalf of Bestie Glam Squad

I agree that Bestie Glam Squad mobile spa has complete ownership of such pictures, etc. Including the entire copyright and may use them for any purpose consistent with Bestie Glam Squad mobile spa services. These include but are not limited to reprints, publications, advertisements, social media and any promotional materials in any medium now known or not later developed, including the internet. I acknowledge that I will not receive any compensation for the use of such pictures and hereby release Bestie Glam Squad mobile spa from any and all claims which are in any way connected with such use.  I have read and understood this consent and release.

Make Up Consent Form

Bestie Glam Squad mobile spa requires you the parent to print and sign this waiver. As part of the spa experience age appropriate make up and spa treatment products will be applied this includes, but is not limited to lip gloss, eye shadow, and blush.  Make up products used are hypoallergenic and make up applicators are new and disposable.  I acknowledge that any of the beauty or spa treatment is for entertainment purposes only and that not all Bestie Glam Squad employees are not licensed cosmetologists. Bestie Glam Squad mobile spa will not cut, file, trim, push back cuticles or do any services provided at a certified nail salon.  Bestie Glam Squad mobile spa is an event planning company, and the spa party themes are for entertainment only.

                                         Food Allergy Permission Slip

Your child will be participating in various activities on Bestie Glam Squad.  Additionally, some of the activities involve food products as part of the experience. We take every precaution on Bestie Glam Squad mobile spa to ensure that your child is safe.  Bestie Glam Squad mobile spa will not be responsible for any allergic reactions or life-threatening incident including death that could occur.  Therefore, Bestie Glam Squad mobile spa would appreciate if you would complete and return this consent/permission slip before your child can participate in any food related activities.  If your child has any known food allergies that may prevent them from participating in these activities, we will provide an alternative option per your request. 

                                    

   Liability:

We strictly provide entertainment. We cannot be held responsible for accidental damage caused by your child. Bestie Glam Squad mobile spa assumes no responsibility or liability for accidents or damage caused during the party. In the event of damage or loss of our inventory, the responsibility lies entirely on the client and or guardian. Bestie Glam Squad mobile spa is not responsible for allergic reactions and or any medical injuries from food, beverages, costumes, or any activity taking place before during or after the party.  I hereby release and agree to Bestie Glam Squad mobile spa harmless from and waive on behalf of myself, my heirs, and any personal representatives of any and all causes action, court claims, court claims fee, demands, damages, cost, expenses, and compensation for damage or loss to myself and or property that may be caused by an act, or that may otherwise arise in any way in connection with any services received from Bestie Glam squad mobile spa. I understand that this release discharges Bestie Glam squad mobile spa from any liability or claim that I, my family, and or personal representatives may have against the business with respect to any bodily illness, death, medical treatment, or property damage that may arise from or in connection to any services received from Bestie Glam squad mobile spa. This liability waiver and release extends to the Bestie Glam squad mobile spa together with all owners, partners, and employs.
